NATIONAL ASTHMA GUIDELINES UPDATED IN EXPERT PANEL REPORT 3 - The National Asthma Education and Prevention Program this month issued the first comprehensive update in a decade of clinical guidelines for the diagnosis and management of asthma. The guidelines emphasize the importance of asthma control and introduce new approaches for monitoring asthma. Updated recommendations for managing asthma include an expanded section on childhood asthma, new guidance on medications, new recommendations on patient education in settings beyond the physician's office and new advice for controlling environmental factors that can cause asthma symptoms.

GAPS IN VACCINE FINANCING FOR UNDERINSURED CHILDREN - A study in the current Journal of the American Medical Association finds an increasing number of children are not getting vaccinated against certain childhood diseases. The number of new vaccines recommended for children and adolescents has nearly doubled during the past five years, and the cost of fully vaccinating a child has increased dramatically in the past decade, the researchers report.

EXPANDING COMMUNITY-BASED SERVICES FOR CHILDREN WITH MENTAL HEALTH NEEDS - Increases in emergency use rates for mental health care by children and youth are emblematic of problems with access to community-based services and supports, says a new report from National Center for Children in Poverty examines policy changes that could improve care and expand community-based services for children with mental health needs.

ADDRESSING RACIAL AND ETHNIC DISPARITIES AT THE GROUND-LEVEL - New report on practical strategies being implemented nationwide to address inequalities in care highlights the need for standardized collection of race, ethnicity, and language data; culturally competent approaches; and the participation and commitment of multiple stakeholders.

UCSF STUDY: LOCAL PHARMACIES COULD DO MORE TO COMBAT CARE DISPARITIES - Despite years of effort in reaching out to their local communities, the role pharmacists play as health care providers still remains unclear to the people who need them the most – elderly Americans with multiple medications for chronic diseases. The study also showed that the individual pharmacists, and the profession as a whole, need to make increased efforts to become integral partners of local, state and national initiatives aimed at decreasing health disparities, said the author.

TELEHEALTH GAP NARROWING - Despite fears that the elderly and poor might be missing out on health information on the Internet, a new study shows disparities are narrowing. Online searches for Medicaid and Medicare information were unrelated to a person’s race, income or sex, researchers found. Rural Americans were also avid Web surfers for health information, despite living in places with less broadband access.

STUDIES FIND LOW USE OF PREVENTIVE HEALTH CARE AMONG MINORITIES - Previous studies have found disparities in minority access to preventive care, but most have focused on economic barriers. A study from the University of Texas M.D. Anderson Cancer Center found minorities Used preventive services at the lowest rates, even after adjusting for socioeconomic reasons. Researchers said many believe they're not at risk, don't know about recommended services or doubt the services' effectiveness.

** EMERGENCY MANAGEMENT:

NEW HRSA PINs - The Health Resources and Services Administration issued a Policy Information Notice (PIN) 2007-15 to provide guidance on emergency management expectations for health centers to assist them in planning and preparing for future emergencies. A second PIN 2007-16: Federal Tort Claims Act (FTCA) Coverage for Health Center Program Grantees Responding to Emergencies, clarifies the scope of FTCA coverage for FTCA-deemed Health Center Program grantees during an emergency. Nonprofit organizations are a crucial link in our nation's emergency preparedness and disaster response efforts, but their role is not always well integrated into disaster planning, according to a recent brief from the Urban Institute.

CALIFORNIA PREPARES FOR DISASTER WITH MOBILE HOSPITALS - The state purchased the three mobile hospitals this year, each with 200 beds, a trauma unit, a pharmacy, emergency and operating rooms and X-ray capabilities. Los Angeles County has also purchased a 100-bed mobile hospital.

Rx RESPONSE TO DELIVER MEDS DURING DISASTER - Organizations that manufacture, dispense and distribute pharmaceuticals have launched Rx Response, an online program designed to ensure that medicine delivery is not disrupted during public emergencies.

** FUNDING:

$25 MILLION IN NEW GRANTS FOCUS ON UNDERSERVED, CARE IMPROVEMENTS IN CA -Funded by the merger of UnitedHealth Group and PacifiCare, grants can be used for Technology Improvements, Medical Education, Disease Prevention and Coordinated Care – all focused on underserved populations and improvements in health care delivery. Applications accepted through Oct. 12, 2007 for the first round of grants to be made in December. Three more proposal periods are planned, with the next beginning in January of 2008. CPCA voiced concern that the Department of Managed Health Care did not limit applicants for the grant to safety net providers.

CMSP BEHAVIORAL HEALTH PILOT PROJECT RFP - Under a three-year pilot project, the County Medical Services Program Governing Board seeks to test the effectiveness of primary care driven, enhanced mental health and substance abuse treatment services for selected CMSP members with mental health and/or substance abuse treatment needs. Proposals are due Oct 10.

FUNDING FOR HUMAN REPRODUCTION EDUCATION PROJECts - An initiative of the Lalor Foundation seeks to educate young women about human reproduction in order to broaden and enhance their options in life. The program focuses particularly on young women who have inadequate access to information regarding reproductive health, including the subjects of contraception and pregnancy termination, and as such may be particularly lacking options in their lives. Deadlines for the receipt of concept papers are November 15 and May 15, annually.

HRSA RURAL HEALTH NETWORK DEVELOPMENT PROGRAM & PLANNING GRANTS - Apply by Oct 15, 2007, for HRSA funding to support the development of rural health networks. One year planning grants also available, apply by Oct 5.

UPCOMING CALLS FOR PROPOSALS FROM ROBERT WOOD JOHNSON FOUNDATION - In November, 2007, the Foundation expects to publish two calls for proposals under their Building Human Capital program: Bringing Diversity to RWJF Grantmaking; and Executive Nurse Fellows Program. Also in November, they will call for proposals for Reclaiming Futures: Communities Helping Teens Overcome Drugs, Alcohol & Crime. The foundation also accepts unsolicited proposals throughout the year under three program areas—Building Human Capital, Vulnerable Populations and Pioneer.

NIH GUIDE: FROM GRANT IDEA TO WINNING PROPOSAL - The National Institutes of Health as posted a powerpoint guide that offers tips on going from a grant idea to a winning proposal. The suggestions are applicable to private foundations as well.

** HEALTHCARE POLICY & TRENDS:

BUDGET SIGNED; GOVERNOR'S LINE-ITEM VETO HITS EAPC, MEDI-CAL RESERVE, MENTAL HEALTH - Ending a stalemate that delayed Medi-Cal payments to providers and threatened facilities with closures, the Governor signed the overdue state budget, using his line-item veto authority to cut almost 700 million. More than $527 million of the cuts came from Health and Human Services, including $310 million from California's Medi-Cal reserves. Most critical to community clinics is the unexpected $10 million cut in the Expanded Access to Primary Care (EAPC). With the budget approved, lawmakers refocused on legislation to overhaul California's health care system. With the governor and Democrats still so far from finding common ground, Senate President Pro Tempore Don Perata suggested a special session to continue work on a compromise after the regular legislative session ends Sept. 14.

$61 MILLION IN NEW HEALTH CENTER GRANTS FROM HRSA - HRSA announced grants under the High Poverty County Initiative, and New Access Point awards including California grantees: Avenal Community Health Center, Chinatown Service Center, Los Angeles, Family Health Centers Of San Diego, North East Medical Services, San Francisco, Westside Family Health Center in Santa Monica and Clinicas del Camino Real, Inc. in Ventura.

NEW HEALTH CENTER REPORT - ACCESS GRANTED: THE PRIMARY CARE PAYOFF - To mark Health Center Week in August, National Association of Community Health Centers released a study that finds Community Health Centers not only provide care to over 16 million Americans, but do so while saving money in health care costs and generating substantial economic benefit in some of the neediest communities. It includes specific information about cost savings, economic impacts and job creation.

NEW ORLEANS COULD BE NATIONAL MODEL: COMMUNITY CLINICS AS MEDICAL HOMES - According to USA Today, If the initiative to establish a network of community clinics as "medical homes" for residents takes root, it will become "the first citywide model of a new approach to providing colorblind medical care for poor people in New Orleans and, ultimately, nationwide."

NEARLY HALF OF ADULTS IN NEW ORLEANS REPORT HEALTH CARE ACCESS PROBLEMS POST-KATRINA - Analysis by the Kaiser Family Foundation shows that more than four in 10 (43%) of adults reported at least one health care access problem in the aftermath of Hurricane Katrina. Underscoring the racial disparities documented generally in the Kaiser household survey, 70% of the one in four adults without health insurance in Orleans Parish were African Americans. In Orleans Parish, the survey found that 33%of African American adults were uninsured versus 12% of white adults.

WHITE HOUSE MOVES TO RESTRICT ELIGIBILITY FOR SCHIP - The Bush administration has outlined new standards for SCHIP enrollment that aim to limit coverage to the lowest-income children, an example of the White House "taking action on its own to advance policies that have not been embraced by Congress," according to coverage by the New York Times. Legislation (HR 3162) approved by the House would increase funding for SCHIP by about $50 billion over five years, and make a number of revisions to Medicare. The Senate version (S 1893) would boost funding for the program by $35 billion over five years. President Bush, who has said he would veto the House and Senate bills, has proposed a $5 billion increase over five years for SCHIP. Without sufficient SCHIP funding, California will not be able to cover the 800,000 children who rely on the Healthy Families Program, much less reach more uninsured children.

MANY CALIFORNIANS STRUGGLE TO PAY FOR HEALTH CARE; POLL FINDS MORE FAVOR SINGLE-PAYER SYSTEM - An August, 2007 report from the California Budget Project and the UCLA Center for Health Policy Research finds many families spend a substantial amount on health care premiums and out-of-pockets costs, and could face financially devastating medical expenses if they are not adequately protected. As voter dissatisfaction with the state's health care system grows, increasing interest is emerging in moving to a state-run, single-payer program, according to a Field Poll released on this month. Only 28% of those surveyed were satisfied with health care in California; 69% want reform, but just 6% thought it very likely that Sacramento would act before the legislative session ends Sept 14.

STUDY: LESS THAN HALF OF AMERICANS ARE HAPPY WITH THEIR HEALTH CARE - According to a report released this month by the Agency for Healthcare Research and Quality, only 48% of Americans age 18 and over who had gone to a doctor or medical clinic within a year of being surveyed rated their health care excellent.

$6 MILLION HEALTH REFORM CAMPAIGN AIMS TO PUSH SACRAMENTO INTO ACTION - The California Endowment, California’s largest health care foundation, says the health care system is in “critical condition” and declared a “code blue” as it unveiled a hard-hitting new TV ad, part of its $6 million paid advertising effort to urge the state’s leaders to enact health care reform this year.

CALIFORNIASPEAKS OUTCOME: LAWMAKERS VOW HEALTH REFORM THIS YEAR - Nearly 3,500 Californians gathered in eight cities on August 11, 2007. During the interactive forum, 82% advocated major changes; the most popular suggestions called for putting people before profit, prioritizing wellness and prevention, and making health care affordable and accessible to everyone.

CA HEALTH REFORM: WHAT'S AT STAKE FOR RURAL COMMUNITIES? - California State Rural Health Association has published a policy brief that examines how several of the major health care reform proposals, including the Governor's, would affect health care for millions of California's rural residents.

SAN FRANCISCO HEALTH ACCESS PROGRAM AN IMMEDIATE HIT - Coping with the overwhelming interest in the Healthy San Francisco program has been the only major hurdle so far, said Dr. Mitch Katz, the city's public health chief. After a limited launch in July, 22 clinics will begin enrollling residents city-wide on Sept 17. The city selected Plexis Healthcare Systems products for benefits administration, electronic data interchange, and analytic processing and reporting for the program.

AHRQ SEEKS IDEAS FOR HEALTH CARE INNOVATIONS EXCHANGE - The Agency for Healthcare Research and Quality is seeking industry ideas to feature in a searchable repository of activities and tools designed to accelerate the development and adoption of innovations in health care. AHRQ plans to make the Innovations Exchange available next spring.

WHY GAMES MATTER: A PRESCRIPTION FOR IMPROVING HEALTH AND HEALTH CARE - A new competition seeks novel ways that video and computer games can be used to help people manage their health and improve how their care is provided. The online competition, sponsored by the Robert Wood Johnson Foundation, uses a unique, open source competition model that promotes enterprising solutions to social problems. Submit competition entries by September 26.

This report brief focuses on the top lessons learned for successful IT collaboration and offers issues to consider when designing or implementing similar projects.

HRSA AWARDS $31.4 MILLION IN HEALTH IT GRANTS TO CHCS AND SAFETY NET PROVIDERS - In California, Redwood Community Health Coalition of Santa Rosa was awarded an EHR Initiative grant. Health Information Technology Planning Grants were awarded to South Central Family Health Center of Los Angeles, Asian Health Services in Oakland and Clinicas Del Camino Real in Ventura. Community Health Center Network in Alameda, the Association of Asian/Pacific Community Health Organizations in Oakland, and the Council of Community Clinics of San Diego won Health Information Technology Innovation Initiative grants.

CALRHIO AWARDS HIE GRANTS TO SAFETY NET ORGANIZATIONS - Through a grant from Blue Shield of California Foundation, CalRHIO is funding four safety net to strengthen, advance, or expand existing health information exchange initiatives aimed at improving clinical efficiency, quality of care, and patient safety. The awardees are: Alliance Medical Center, Healdsburg; Community Health Partnership, Santa Clara ; San Francisco Community Clinic Consortium and Tehachapi Valley Health Care District.

LESSONS LEARNED FROM THE SANTA BARBARA COUNTY CARE DATA EXCHANGE - A new report from California HealthCare Foundation provides a comprehensive, independent evaluation of the factors leading to the closure of the eight-year-old RHIO in Santa Barbara County. The authors cite lack of a compelling business case, distorted economic incentives, passive leadership among participants, vendor limitations, software delays, and privacy and security issues as factors that played a significant role in the project's eventual closure.

PRIVACY AND SECURITY SOLUTIONS FOR RHIOs - The Agency for Healthcare Research and Quality this month released a set of reports that review 34 state Health Information Exchange plans and identify the challenges and feasible solutions for ensuring the safety and security of electronic health information exchange.

ONC RECOMMENDATIONS FOR EHRS INCLUDE CONTROVERSIAL ANTI-FRAUD REQUIREMENT - A report released this month by the Office of the National Coordinator for Health Information Technology makes 14 recommendations to improve existing standards for electronic health record systems, and create additional requirements to improve oversight of billing accuracy and detect potential improper payments. It includes a controversial call for requirements that EHRs be designed to provide payers, acting as fraud auditors, remote access to patient records. A brief 14-day comment period garnered no comments by privacy advocacy organizations.

EMR SURVEY FINDS MALPRACTICE BENEFITS, NEED FOR MORE TRAINING AND FUNCTIONALITY - In a survey from MRI, about a quarter of those who have EMR systems complained the system does not have the functions they need, and a similar percentage stated they did not receive adequate training. About 20% of respondents said their malpractice insurer offers discounts for EMR systems and more than half found their EMR to be helpful in a malpractice case.

e-HEALTH AND AMERICA'S BROADBAND NETWORKS - A study identifies four legislative and regulatory goals that must be achieved to support the rapid advance in eHealth services through advances in broadband: Adoption of Public-Private Partnerships; Reform of the Rural Utility Service loan program; Incentives for adoption of administrative and clinical solutions; and Rejection of “network neutrality,” a system in which certain content providers would pay more to expedite their traffic on the Internet.

MEDICARE:

PROGRESS REPORT: MEDICARE D EXPANDED COVERAGE FOR SENIORS, BUT MANY FIND DRUGS TOO EXPENSIVE - The Medicare prescription drug benefit has significantly increased drug coverage for beneficiaries, but one in five reported delaying or not filling prescriptions because of cost. Researchers say that 3.4 million to 4.7 million beneficiaries are eligible for, but do not receive, low-income subsidies that would help cover costs, and about half of eligible low-income seniors not receiving these subsidies were aware of their availability. Because of a combination of subsidy decreases and premium increases, 1.6 million low-income individuals will no longer have a sufficient subsidy to cover the premiums of their existing plans, leaving them the choice of paying more or changing plans.

** QUALITY:

CARE QUALITY IMPROVEMENT THROUGH BETTER COMMUNICATION, COORDINATION - New analysis from Lumetra, California’s Quality Improvement Organization, focuses on ways healthcare information technology (HIT) increases communication among providers, with staff, and between patients and physicians. An evaluation of best practices for improving communication between Hispanic patients—particularly the elderly—and physicians recommends six strategies, including the importance of improving trust and respect, addressing language and medical literacy barriers, and improving access to care.

NEW CENSUS DATA: RECORD NUMBER IN U.S. AND CALIFORNIA LIVING WITHOUT HEALTH INSURANCE - The latest data on poverty, income and health insurance reports a slight drop in poverty, but adults over age 65 drove most of this decline -- poverty rates among children and working-age adults saw no significant change, and are still far above 2001 levels. The number of uninsured increased for the sixth year in a row, rising to a record 47 million (up 2.2 million since 2005). And the number of uninsured children increased from 8 million (10.9 percent) in 2005 to 8.7 million (11.7 percent) in 2006. In California, nearly one in five of the state's residents went without health insurance at some time during 2006; 1.2 million children in the state do not have health insurance.

MOST YOUNG KIDS GET PREVENTIVE CARE - The latest edition of HRSA’s Child Health USA reports that 85.3 % of children under four years of age had at least one preventive medical visit in 2004. Early prenatal care among racial and ethnic groups with historically low rates of utilization, including non-Hispanic Black, Hispanic, and American Indian/Alaska Native women, has increased by at least 20 percent since 1990.

OBESITY EPIDEMIC CONTINUES TO GROW - Adult obesity rates rose in 31 states last year, according to the fourth annual F as in Fat: How Obesity Policies are Failing in America, 2007 report from the Trust for America's Health (TFAH). Twenty-two states experienced an increase for the second year in a row; no states decreased. A new public opinion survey featured in the report finds 85 percent of Americans believe that obesity is an epidemic. California ranked #37 among states in adult obesity, and 33rd in child obesity.

OBESITY & DIABETES - A new policy brief by the UCLA Center for Health Policy Research shows that the number of Californians diagnosed with diabetes is continuing to rise, and reveals that rates vary considerably as one moves throughout the state. Two recent UCLA publications funded by The California Endowment address the issues of obesity and diabetes.

GOVERNOR ANNOUNCES FUNDS FOR PHYSICIAN TRAINING -To meet the growing demand for health care practitioners throughout California, Governor Schwarzenegger this month awarded nearly $2.5 million to 26 family practice residency training programs. The awards are administered through the Office of Statewide Health Planning and Development’s (OSHPD) Song-Brown program. This fall, the California Healthcare Workforce Policy Commission will be considering applications for Song-Brown Program Grants for the Family Nurse Practitioner Training Programs and Physician Assistant Training Programs.

UC-DAVIS TO OPEN NURSING SCHOOL IN 2008 - A $100 million donation from the Gordon & Betty Moore Foundation will establish a nursing school at the university's Sacramento campus.
